How To Place An Order

Ordering Online
To make a purchase online, simply select your product.  Personalize your order by selecting: size, color and any desired add-on options click the "Buy Now" button to add your item(s) to the shopping cart. To place an order for 'design your own' custom garments currently not available in the online boutique. Simply complete and submit a price quote request here. A detailed quote will be provided with a secure personal link. Access the link provided within the quote to begin the ordering process.

Step 2 > Checking Out

Once finished shopping, click on the "Proceed to Checkout" button to finalize your order and complete payment. If you encounter problems when making your online purchase, contact us for assistance.



Step 3 > Completing Payment
At the payment page, confirm accuracy of all order details. Complete the billing and shipping details as requested. Finalize, your order by submitting any applicable coupons or store discount.  Select preferred method of payment and shipment. To purchase, select the 'Place Your Order' button at the bottom of the page to complete. Upon order completion an electronic receipt will be provided via email. *Please print this form in full if completing an order by mail or fax.



Ordering By Mail or Fax: 
Order by Fax - Diva by Design also accepts orders via fax or mail. Simple complete the process above. Fax the completed payment form with payment details to Attn: New Orders Fax: 208-694-1057.
